Forest scored all their penalties through Morgan Gibbs-White, Ibrahim Sangare, and Neco Williams, while Midtjylland missed three attempts.
During the match, Ryan Yates scored a key goal, but Midtjylland forced extra time. Forest eventually held their nerve to win the shootout.
They will now face Stuttgart or Porto in the next round.
